REGISTRATION CHECKLIST

CHECK-IN PROCEDURE Teams shall be required to register on Friday, November 13th at the La Quinta Inn & Suites from 6-10 pm.  Saturday morning check-in will be permitted with approval from the tournament director but must be completed by 9:00 AM Saturday. 

The following documents shall be required for each team to be properly registered for the tournament:

1) State Association Certified Team Roster (3 copies)

2) State Association issued Player Pass for each team member with photo ID

3) Coach Pass for each coach or manager that will be on sideline with team

4) Individual Medical Release for each player

5) USYSA Permission to Travel or Intent to Travel Form (for non-Alabama teams, if required by your state association)

6) Loan form for guest players (if applicable)

After being properly registered, each team representative will be given a team packet that includes a  game schedule for that team’s respective bracket, a copy of the tournament rules, and other miscellaneous documentation. Each team representative will be required to sign a “Team Check-In Sheet” to confirm that he/she understands all rules and requirements for participating in the tournament.
